Job Summary:
Home Health Care Nurse (RN) Portland, Maine. This role involves providing comprehensive home health care services to adult patients across various medical conditions, including cardiac, neurological, and respiratory needs. The position offers a daytime schedule with a focus on patient-centered care, along with flexible rotation options to accommodate agency needs.

Job Details:

• Schedule: Day shift from 0800 to 1700 hours, 40 hours per week.
• Required Qualifications: Valid RN licensure, minimum of 2 years of experience, BLS certification, proficiency with Epic and OASIS electronic health record systems, and current CPR certification.
• Preferred Qualifications: Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, s/p catheters, laboratory draws, and home health care settings.
• Float: No float rotation may be required.
• Additional Information: The position involves managing 20-25 patients as a case manager, with an expectation of 5-6 visits daily. Clinicians will be equipped with essential electronic devices and supplies, including a laptop, I-phone, vital signs equipment, INR machine, and nursing supplies. Call-back duties are minimal, with no overnight call and 2-4 callback incidents per month between 1700-2000 hours. The position is a replacement for permanent staff and begins on 07/27/2026.
