Job Summary

Nurse, Medical-Surgical Floor. The position provides in-home nursing visits and supports patient enrollment in a hospital-at-home program. The role includes daytime shifts, rotating weekend coverage, and occasional overnight on-call responsibilities.

Job Details

Schedule: Day shifts consisting of 12-hour shifts, generally rotating between 7:00 a.m.7:00 p.m. and 9:00 a.m.9:00 p.m., with a minimum of 36 hours per week.
Assignment Dates: 12/07/202603/06/2027.
Additional Information: Responsibilities include in-home patient visits and functioning as an enrollment nurse within the healthcare facility. Every-other-weekend coverage and approximately one to two overnight on-call shifts per week may be required, with infrequent call activity anticipated. Orientation to the healthcare facilitys units and the hospital-at-home care team will be provided.
