We are seeking an experienced LVN/LPN Case Manager for a travel/contract position in Rochester, NY. This full-time role requires an 8-hour day shift schedule, Monday through Friday, with guaranteed 40 hours per week. The position is mobile and includes travel to various locations such as hospitals, patient homes, and provider offices. Responsibilities include appointment scheduling, biometric data monitoring, patient education, care coordination, and connecting patients with community resources. You will work closely with a multidisciplinary team that includes RNs, Social Workers, and Pharmacists to support a Transitions of Care program aimed at reducing 30-day readmission rates. This is not a hands-on nursing role but requires clear communication, strong documentation skills, and the ability to work independently. Preferred candidates will have experience with CHF, COPD, and managing multiple complex comorbidities. Reliable transportation and flexibility are essential.
