LPN – Case Manager | Travel Contract📍 Location:  Rochester, NY 📅 Start Date: 11/02/2026 | Duration: 13 weeks 🕗 Shift: Days, Monday–Friday, 8:00 AM–4:30 PM ⏰ Hours: 40 hours/week  Requirements:<ul><li>Minimum 1 year of LPN experience</li><li>Active NY LPN license in hand</li><li>BLS required</li><li>EPIC experience required</li><li>Home health, case management, or clinic experience preferred</li><li>Reliable transportation required</li></ul>Role Details:<ul><li>Care coordination and transitions-of-care support</li><li>Patient follow-up calls, appointment scheduling, and medication education</li><li>Coordinate community resources and reinforce care plans under RN direction</li><li>Mobile role with potential home, hospital, and provider-office visits</li><li>Adult/geriatric patient population; no hands-on nursing care</li><li>No weekends; floating may be required</li></ul>Important: Candidates must live more than 75 miles from the facility. First-time travelers are welcome.
