RN Case Manager
StartDate: 5/25/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 1 year case management experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7:00AM - 7:00PM Sat-Mon

SYSTEMS – Meditech

START DATE – ASAP
