POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge planning, Chart review and patient screening, Care Coordination, Barrier mitigation in an acute hospital setting

MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license; BLS; 2 years case manager experience in an acute hospital setting; Acute hospital experience during the past 12 months with no more than a 90-day break

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:30A-5:00P. Potential rotating weekends.

SYSTEMS – Meditech, Midas

START DATE – ASAP
