POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS: 2 years case management experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00am-4:30pm M-F and every other weekend

SYSTEMS – Meditech, Midas, Wellsky

START DATE – ASAP
