POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; 2 years acute hospital case management experience; recent California Dignity Health Hospital experience as a Care Coordinator

LENGTH OF ASSIGNMENT – 13 Weeks

SHIFT / HOURS PER WEEK – 8hr Days, EOW

SYSTEMS – Cerner

START DATE – 6/9/2026
