POSITION SUMMARY – RN Care Coordinator

POSITION DUTIES – Oversee and streamline patient care by assessing needs, coordinating interdisciplinary services, facilitating communication among providers and families, and ensuring safe, efficient transitions throughout the hospital stay.

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; ACLS; 2 years recent RN Care Coordinator experience in an acute hospital setting; 2+ years minimum of acute care hospital experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 7a-7p, EOW

SYSTEMS – Cerner

START DATE – ASAP
