Minimum 3 years recent RN experience in Care Coordination / Case Management (acute care setting required; must have solid care coordination experience) / MUST have prior Dignity Health / CommonSpirit Health experience (REQUIRED – no exceptions); strong communication and coordination skills required ./ CERTIFICATIONS – BLS Required; LA City Fire & Safety Required / LICENSE – Active CA RN License Required / Schedule: Days, 8-hour shifts, 40 hrs/week, 13 weeks (every other weekend + first to float) / Setting: Acute Care – Case Management / Care Coordinator / EMR: Cerner / Skills: Care coordination, discharge planning, utilization review, interdisciplinary collaboration, patient advocacy, transition of care management / Floating: Required; first to float / Support: Interdisciplinary care team environment / Scheduling: Facility scheduling; flexibility required / Orientation: 12 hours
