RN Case Manager
StartDate: ASAP Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY Case Manager - Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med.

POSITION DUTIES  Role: Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker. Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians/team. Need to have a positive attitude, be able to critically think, and good time management skills. Utilization Review (UR) is a separate dept- UR will do insurance approvals and arranging transportation

MINIMUM REQUIRED QUALIFICATIONS  Min 1 year experience; BLS Certification, PRI (Patient Review Instrument) Certification, SCREEN Training Certification
 PRI & SCREEN certifications (can be taken online through the School of Albany- typically take a couple hours for each certification; Cost approx. $300/certification) - />
