Registered Nurse – Home Health- Hospice - Travel - (HH RN)

This Home Hospice RN position is based in the Alpharetta, Georgia area, a sought-after northern suburb known for its welcoming neighborhoods, excellent schools, and exceptional quality of life. Alpharetta offers beautiful parks and greenways, family-friendly events, and a thriving town center with specialty shops, restaurants, and entertainment venues. Your work will be highly meaningful, focused on building relationships, promoting quality of life, and honoring patient and family goals. Typical responsibilities include performing comprehensive initial and ongoing assessments in the home environment, developing and updating individualized plans of care, and coordinating services within an interdisciplinary hospice team. You will manage pain and other symptoms through appropriate interventions, administer and monitor medications, and educate patients and families about what to expect as illness progresses. Frequent communication with physicians, social workers, spiritual care providers, hospice aides, and other team members will ensure a cohesive and responsive plan of care. On a typical day, you will travel throughout Alpharetta and nearby communities to provide scheduled and as-needed visits. Your caseload will consist of home-based hospice patients whose needs range from routine symptom management to complex care requiring close follow-up. You will triage concerns, respond to urgent changes in condition, and provide emotional support and guidance to families. While patient ratios may vary based on acuity and census, the focus remains on maintaining safe, manageable caseloads that allow for thorough, individualized attention. You can expect a schedule that includes daytime hours with some rotation of on-call or after-hours responsibilities, depending on the needs of patients and the hospice program’s structure. Flexibility is important, as visit times adapt to patient and family schedules and to urgent care needs that arise. The clinical environment is supported by modern technology for documentation and communication, enabling efficient coordination of services and timely updates to the care team. You will be part of a collaborative culture that values professional growth, clinical excellence, and respect for the diverse needs of patients and families. The role offers opportunities to deepen expertise in hospice and palliative nursing, to explore areas of specialization such as complex symptom management, and to contribute to quality improvement initiatives.
