POSITION DUTIES Discharge Planning, Patient Assessment, Care Coordination

MINIMUM REQUIRED QUALIFICATIONS 5+ years clinical nursing experience with 1+ year Case Management experience; CA RN License in Hand, BSN, AHA BLS

PREFERRED QUALIFICATIONS CCM, ACM

LENGTH OF ASSIGNMENT 13 weeks
