Job Title

Physical Therapist (PT) - Home Health

Location

Pomona, CA

Schedule / Job Type

Contract | Full-Time | Onsite | Monday through Friday, 8:00 AM – 5:00 PM | No on-call or weekend requirements

Key Responsibilities

Provide home health physical therapy services to patients in assigned territories including Yorba Linda, Orange, Anaheim Hills, and Santa Ana
Manage a caseload of approximately 6 patient visits per day
Evaluate and treat patients in their home setting, developing and implementing individualized care plans
Document patient progress and maintain accurate clinical records in compliance with home health standards

Required Qualifications

Active Physical Therapist (PT) license in the state of California
Minimum of one year of home health experience
Ability to travel to patient homes within the designated service territories

Preferred Experience / Skills

Experience with HCHB (Homecare Homebase) documentation software
