Job Title

Physical Therapist - Home Health

Location

Marysville, CA

Schedule / Job Type

Contract | Full-Time | Days | Monday - Friday | Onsite

Key Responsibilities

Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
Meet department objectives while delivering high-quality patient care in a home health setting
Serve as co-leader in an interdisciplinary approach to patient care and group treatment
Provide physical therapy services across the Butte, Yuba, and Sutter County service areas

Required Qualifications

Doctor of Physical Therapy (DPT) degree required for those who obtained licensure on or after January 2016
Active Physical Therapy licensure in all applicable states of practice
Current CPR or Basic Life Support (BLS) certification
Valid Driver's License; must be at least 21 years of age or older

Preferred Experience / Skills

Minimum of 1 year of prior home health experience preferred
Experience with Home Care Home Base (HCHB) software
