Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute evaluations and 30-minute follow-up sessions; progress patients toward discharge within an established length of stay
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing supervision, learning objectives, and student evaluations
- Adhere to discipline-specific State Practice Act and regulatory requirements; actively pursue professional development through continuing education and professional organizations
Required Qualifications
- Active and unrestricted Physical Therapist license in good standing in the state of Colorado
- Experience performing comprehensive PT evaluations and developing individualized treatment plans
- Proficiency in medical record documentation in compliance with state and federal guidelines
- Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct
- Commitment to patient and employee safety standards including safe patient handling and infection control practices
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring PT students in a clinical environment
- Familiarity with performance improvement methodologies and quality of care initiatives
