Physical Therapist

Schedule / Job Type

Contract (13 Weeks) | Days | Monday through Friday | Onsite

Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans
Deliver hands-on therapeutic interventions to support patient recovery and functional improvement
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to ensure coordinated patient care
Maintain accurate and timely clinical documentation in accordance with facility standards

Required Qualifications

Active Physical Therapist (PT) license in the state of New Hampshire
Graduate of an accredited Physical Therapy program
Ability to meet facility compliance requirements, including valid state or federal issued photo identification
Must adhere to facility badging and identification policies at all times while on site

Preferred Experience / Skills

Prior experience in a skilled nursing or long-term care setting
Familiarity with geriatric patient populations and related rehabilitation needs
