Physical Therapist – Home Health

Location

San Francisco, CA (and surrounding areas including Brisbane, Daly City, Colma, South San Francisco, Pacifica, Millbrae, Hillsborough, Burlingame, San Mateo, and neighboring cities)

Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs with the goal of restoring function and minimizing disability following disease or injury
• Conduct initial admission and evaluation visits, completing and reporting OASIS scores to the Clinical Team Manager within established timelines
• Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans in accordance with established standards
• Supervise Physical Therapy Assistants as applicable
• Help patients achieve maximum performance and function within the limits of their capabilities
• Perform additional duties consistent with skills, training, and the goals of the home health program

Required Qualifications

• Active Physical Therapist license in the state of California
• Minimum of one year of home health experience
• Proficiency in evaluating and treating patients under medically prescribed physical therapy programs
• Familiarity with OASIS documentation and reporting requirements
• Ability to travel throughout an assigned territory covering multiple cities in the San Francisco Bay Area

Preferred Experience / Skills

• Experience supervising Physical Therapy Assistants in a home health setting
• Strong documentation and clinical assessment skills within a home-based care environment
