Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Ensure care is provided under signed, valid physician orders and within insurance approvals as required for patient care
• Supervise and mentor students as assigned, providing a positive learning environment, developing learning objectives, and completing student evaluations
• Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels

Required Qualifications

• Active and unrestricted Physical Therapist license in good standing in the state of Arizona
• Adherence to the state-specific Physical Therapy Practice Act and all applicable regulatory requirements
• Completion of all state-mandated continuing education requirements
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

• Prior experience in outpatient orthopedic physical therapy
• Experience with clinical student supervision and mentorship
• Familiarity with functional quality indicators and medical necessity documentation standards
