Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
- Conduct 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting
- Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Design functional treatment programs that support effective patient return to community activities and progress patients toward discharge within an established length of stay
- Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements; maintain professional licensure in good standing
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations
Required Qualifications
- Active and unrestricted Physical Therapist licensure in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct
- Ability to complete thorough clinical documentation in compliance with state, federal, and payer guidelines
- Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Active involvement in continuing education and professional development activities
