Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Perform outpatient evaluations (60-minute initial evaluations) and follow-up visits (30-minute sessions), progressing patients toward discharge within an established length of stay
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and regulatory requirements, maintain professional licensure in good standing, and actively pursue continuing education and professional development

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Colorado
• Experience in outpatient physical therapy settings
• Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
• Ability to work across multiple clinic locations as assigned
• Commitment to patient safety and adherence to safe patient handling practices

Preferred Experience / Skills

• Experience with functional quality indicator assessments and letters of medical necessity
• Prior experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies in a clinical setting
