Contract | Full-Time | Onsite | Day Shift | 5 x 8-hour shifts, 40 hours per week

Key Responsibilities

Provide direct physical therapy services to patients in an outpatient setting
Evaluate, plan, and implement individualized treatment programs based on patient needs
Complete required clinical documentation accurately and in a timely manner
Collaborate with the clinical team to ensure continuity and quality of patient care
Maintain compliance with facility credentialing and documentation requirements prior to and during assignment

Required Qualifications

Active Physical Therapist license (permanent state license required)
Prior experience in an outpatient physical therapy setting
Ability to meet contract credentialing and quality assurance requirements
Willingness to complete any facility-specific documentation or onboarding requirements before assignment start

Preferred Experience / Skills

Experience working in a high-volume outpatient rehabilitation environment
Familiarity with outpatient documentation platforms and electronic medical records
