Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans
Provide direct patient care in a skilled nursing facility setting
Monitor and document patient progress toward therapeutic goals
Collaborate with interdisciplinary care teams to coordinate patient treatment
Educate patients and caregivers on therapeutic exercises and home programs

Required Qualifications

Active Arizona Physical Therapist license, or proof of application with an estimated approval date
Graduate of an accredited Physical Therapy program
Experience providing physical therapy services in a skilled nursing or rehabilitation setting
Ability to work 5 days per week with a 32-hour weekly guarantee

Preferred Experience / Skills

Prior experience in a skilled nursing facility (SNF) environment
Strong documentation and clinical assessment skills
Experience working within interdisciplinary rehabilitation teams
