Key Responsibilities

  • Provide occupational therapy services to patients in a skilled nursing facility setting
  • Evaluate and develop individualized treatment plans to address patients' functional needs
  • Document patient progress and maintain accurate records using electronic documentation systems
  • Collaborate with interdisciplinary care teams to ensure comprehensive patient care

Required Qualifications

  • Active California Occupational Therapist license
  • Minimum of one year of experience in a skilled nursing facility (SNF) setting
  • Proficiency with NetHealth (Rehab Optima) documentation software

Preferred Experience / Skills

  • Experience working with geriatric or post-acute patient populations
  • Strong communication and team collaboration skills in a rehab setting
