Key Responsibilities
- Provide occupational therapy services to patients in a skilled nursing facility setting
- Evaluate and develop individualized treatment plans to address patients' functional needs
- Document patient progress and maintain accurate records using electronic documentation systems
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care
Required Qualifications
- Active California Occupational Therapist license
- Minimum of one year of experience in a skilled nursing facility (SNF) setting
- Proficiency with NetHealth (Rehab Optima) documentation software
Preferred Experience / Skills
- Experience working with geriatric or post-acute patient populations
- Strong communication and team collaboration skills in a rehab setting
