Contract | 13-Week Assignment | Skilled Nursing Facility (SNF)

Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
Develop and implement individualized physical therapy treatment plans based on patient needs and goals
Document patient progress, treatment outcomes, and clinical notes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
Educate patients and caregivers on therapeutic exercises and home programs to support continued recovery

Required Qualifications

Active Physical Therapist (PT) license in the state of Oregon or ability to obtain prior to start
Graduate of an accredited Physical Therapy program
Current BLS/CPR certification
Prior experience working in a skilled nursing or long-term care setting

Preferred Experience / Skills

Experience with MDS assessments and PDPM documentation
Familiarity with electronic medical records (EMR) systems used in SNF environments
