Key Responsibilities

Provide physical therapy evaluation and treatment services in a skilled nursing facility (SNF) setting
Deliver patient care across assigned worksites, with openness to floating as needed
Maintain accurate and timely clinical documentation in accordance with facility standards
Collaborate with interdisciplinary care teams to support patient rehabilitation goals

Required Qualifications

Active California Physical Therapist license
Minimum of 1 year of SNF experience
Ability to work 8-hour day shifts, 5 days per week
Willingness to float between worksites as required
Standard credentialing requirements must be met prior to start

Preferred Experience / Skills

Experience with Casamba documentation software highly preferred
Familiarity with multi-site or floating clinical assignments
