Occupational Therapist – Home Health
Location

Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled occupational therapy services to patients in a home health setting across West Portland and surrounding areas
- Conduct patient evaluations and deliver individualized treatment plans with a 1:1 patient ratio, visiting up to 6 patients per day
- Complete Oasis assessments as required for home health documentation standards
- Chart and document patient care using EPIC Remote Client and the Rover mobile application
- Manage daily scheduling efficiently, with occasional travel radius of up to approximately 60 miles per day
Required Qualifications
- Active Oregon Occupational Therapist license
- Minimum of 1 year of skilled home health experience
- Current Basic Cardiac Life Support (BCLS) certification
- Demonstrated experience with Oasis assessments
- Reliable personal transportation required
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills
- Familiarity with EPIC Remote Client for home health charting
- Experience meeting productivity point expectations in a home health environment (standard 30 productivity points)
