Physical Therapist – Float (Part-Time)

Contract | Days | Weekend rotations approximately once every 6 weeks

Key Responsibilities

• Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Serve a diverse patient population ranging in age from birth to over 100 years
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Maintain thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Communicate effectively across interdisciplinary teams to ensure coordinated, high-quality patient care
• Travel to patient cases as assigned throughout the designated service area

Required Qualifications

• Active Physical Therapist professional license in the state of Massachusetts
• Minimum of 1 year of experience within a certified home health agency (certified home care experience only)
• Minimum of 1 year of case management experience within a certified home health setting
• Ability to travel to patient cases as assigned
• Note: Experience in hospice, private care, or case management outside of a certified home health agency does not fulfill the certified home health care experience requirement

Preferred Experience / Skills

• Experience working with patients across a wide range of ages and diagnoses in a home care setting
• Familiarity with electronic medical records and documentation platforms used in home health environments
