Key Responsibilities

• Provide physical therapy services to patients in their place of residence across assigned cases
• Evaluate and treat patients with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Complete thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Participate in interdisciplinary communications to support coordinated patient care
• Participate in weekend rotations on a scheduled basis (approximately once every six weeks)

Required Qualifications

• Active Physical Therapist professional license in the state of Massachusetts
• Minimum of 1 year of experience within a certified home health agency
• Minimum of 1 year of case management experience within a certified home health setting
• Ability to travel to patient cases as assigned throughout the service area
• Note: Experience in hospice, private care, or general case management outside of a certified home health agency does not satisfy the required home health experience

Preferred Experience / Skills

• Experience treating patients across a broad age range, including pediatric and geriatric populations
• Familiarity with electronic medical records and documentation platforms used in home health settings
