Job Title

Physical Therapist – Home Health

Location

Portland, OR (East Portland and Clark County, WA and Surrounding Areas)

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

Provide skilled physical therapy services to home health patients in a 1:1 care model, visiting up to 6 patients per day
Complete OASIS assessments and documentation accurately and in a timely manner
Chart using EPIC Remote Client for Home Health and the Rover mobile application
Manage a daily patient caseload across a defined geographic service area, with occasional drive times up to 60–90 minutes per day
Maintain 30 productivity points per standard shift in accordance with program expectations

Required Qualifications

Active Physical Therapist licensure in both Oregon (OR) and Washington (WA) required prior to start
Minimum 1 year of skilled home health experience
OASIS experience required
Current Basic Cardiac Life Support (BCLS) certification
Must have reliable personal transportation

Preferred Experience / Skills

Experience with EPIC Remote Client for Home Health documentation
Familiarity with the Rover mobile charting application
Ability to efficiently manage a geographically distributed patient caseload
