Contract | Full-Time | Onsite | Day Shift | 8-hour shifts, 5 days per week, 2 weekends per month
Key Responsibilities
- Deliver high-quality, patient-centered physical therapy services across an Acute Inpatient Rehabilitation Unit and a Skilled Nursing Facility (SNF)
- Evaluate and treat patients recovering from neurological injuries and surgeries in an acute inpatient setting
- Provide geriatric rehabilitation services to elderly patients with diverse rehabilitation needs in the SNF
- Collaborate with multidisciplinary teams to develop and implement individualized treatment plans aimed at improving functional outcomes
- Maintain accurate and thorough documentation using Epic EMR
- Float between the Inpatient Rehabilitation Unit and Skilled Nursing Facility based on unit needs
Required Qualifications
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Active California Physical Therapist license (or eligibility for licensure)
- Current CPR/BLS certification
- Strong knowledge of neurological disorders and evidence-based rehabilitation techniques
- Excellent communication, interpersonal, and problem-solving skills
- Must meet facility nicotine-free requirements; negative nicotine/cotinine result required
Preferred Experience / Skills
- Experience in neurological rehabilitation or acute inpatient care
- Experience in geriatric rehabilitation and SNF care
- Familiarity with Epic EMR
