Job Title

Speech Language Pathologist (SLP)

Schedule / Job Type

Contract | Full-Time | Days | Onsite / Field-Based Home Health

Key Responsibilities

Provide speech-language pathology services to home health patients across an 11-county territory spanning two office locations
Evaluate and treat patients with diagnoses including CVA and Aphasia
Manage a caseload of approximately 6 patients per day in a home health setting
Travel throughout the assigned service territory to conduct in-home patient visits
Maintain professional documentation and reporting using provided phone and tablet technology
Participate in a two-week department-specific orientation with a designated work coach

Required Qualifications

Active Kentucky state license in hand at time of submission — no pending licenses accepted
CCC-SLP credential required
Current BLS certification required
Minimum of 1 year of clinical experience
Prior home health experience required
Ability and willingness to travel across an 11-county territory, with corner-to-corner travel exceeding 2.5 hours

Preferred Experience / Skills

Experience treating CVA and Aphasia patient populations
Familiarity with home health documentation and care coordination workflows
