Job Title

Physical Therapist (PT)

Location

Louisville, KY

Schedule / Job Type

Contract | Full-Time | Days | Onsite | 5 x 8-hour shifts per week, including rotating weekends and holiday rotation

Key Responsibilities

Perform Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily patient visits in a home health setting
Manage a caseload of approximately 6.5 points per day within the assigned coverage area
Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
Travel to patient homes within the designated service area to deliver skilled physical therapy services
Participate in rotating weekend shifts (8:00 AM – 5:00 PM) and holiday rotation as scheduled

Required Qualifications

Active Physical Therapist (PT) license in the state of Kentucky
Current BLS certification
Ability to perform home health visits independently in the field
Willingness to participate in weekend and holiday rotation

Preferred Experience / Skills

Prior home health experience preferred
Experience treating patients with CHF, orthopedic, or pulmonary diagnoses
Candidates with at least some clinical experience beyond new graduation are preferred
