Key Responsibilities

Provide physical therapy services to patients in a home care setting
Evaluate and assess patient conditions to develop individualized treatment plans
Implement therapeutic interventions to improve patient mobility, strength, and function
Document patient progress and treatment notes accurately within the EMR system
Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

Active Physical Therapist license in the state of Ohio
Experience in a home care setting
Proficiency with Epic EMR
Strong clinical assessment and documentation skills

Preferred Experience / Skills

Prior contract or travel therapy experience
Strong communication and patient education skills
