Occupational Therapist (OT) – Home Health

Schedule / Job Type

Contract | Days | Includes weekend rotation (Saturday and Sunday as applicable)

Key Responsibilities

• Assess, plan, treat, evaluate, and implement a Plan of Care (POC) for all assigned home health patients
• Practice under AOTA standards of care and code of ethics, collaborating with physicians and the broader healthcare team
• Provide supportive supervision of OTAs and OT/OTA students
• Complete timely and accurate documentation of all patient care activities in the electronic medical record
• Maintain the highest standards of patient privacy, confidentiality, and safety in accordance with established conduct standards
• Apply evidence-based interventions and pursue continuing education to promote quality of care and professional development

Required Qualifications

• Minimum of 1 year of Home Health Occupational Therapy experience
• Active Illinois Occupational Therapist license at time of submission
• BLS certification (American Heart Association) – must be active at time of submission
• Valid driver's license and current auto insurance
• COVID-19 vaccination required (declinations will not be approved)

Preferred Experience / Skills

• OASIS and start-of-care experience strongly preferred
• Familiarity with home health admission processes, treatment documentation, and appropriate billing practices
