Contract | 13-Week Assignment | Skilled Nursing Facility (SNF) | 35-Hour Weekly Guarantee

Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
Develop and implement individualized physical therapy treatment plans based on patient needs and goals
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to ensure coordinated and effective patient care
Educate patients and their families on therapeutic exercises and home safety strategies

Required Qualifications

Active Physical Therapist (PT) license in the state of Virginia or compact licensure eligibility
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a skilled nursing or long-term care setting

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with MDS documentation and Medicare compliance in SNF environments
