Contract | Days | Friday – Monday, 10-Hour Shifts

Key Responsibilities

Provide outpatient physical therapy services to patients
Evaluate, assess, and develop individualized treatment plans
Float between two clinic locations as needed
Document patient progress and maintain accurate clinical records in accordance with facility standards

Required Qualifications

Active Physical Therapist (PT) license in the state of Oregon or eligibility to obtain
Graduate of an accredited Physical Therapy program
Ability to work a Friday through Monday, 10-hour day schedule
Ability to meet compliance requirements prior to start date

Preferred Experience / Skills

Prior outpatient physical therapy experience
Flexibility and comfort working across multiple clinic locations
