Job Title

Speech Language Pathologist

Location

Gloucester, VA

Schedule / Job Type

Contract | Days | 5x8 Hours (8:00 AM – 4:30 PM)

Key Responsibilities

Provide speech-language pathology services to patients in a home health setting
Complete OASIS assessments accurately and in accordance with home health requirements
Develop and implement individualized treatment plans based on patient evaluations
Document patient progress and clinical notes using required charting systems
Collaborate with interdisciplinary care teams to coordinate patient care
Float to other units or locations based on facility needs as requested

Required Qualifications

Active Speech Language Pathologist license; Virginia license required to be in hand at time of submittal or valid compact license with proof provided
OASIS experience required
Home health experience required
Specialty experience within the last year required
BLS certification through the American Heart Association (AHA) required
SSN required at time of submittal

Preferred Experience / Skills

Experience with EPIC electronic health records system
Prior experience in a travel or contract therapy role
Strong communication and patient education skills across diverse patient populations
