Physical Therapist (PT) – Home Health

Location

Walla Walla, WA

Schedule / Job Type

Contract (13 Weeks) | Full-Time | Onsite | Monday – Friday, 8:00 AM – 5:00 PM

Key Responsibilities

  • Provide skilled physical therapy services to patients in a home health setting across an assigned geographic territory
  • Collaborate with an interdisciplinary team and reporting manager to coordinate and deliver patient care
  • Conduct evaluations, develop individualized treatment plans, and monitor patient progress
  • Travel throughout the assigned territory, which includes portions of Walla Walla County, lower Columbia County, and Umatilla County (up to 60 miles)

Required Qualifications

  • Active Washington State Physical Therapist license (required at time of start)
  • Oregon Physical Therapist license required (candidates may begin with WA licensure while actively obtaining OR licensure)
  • Minimum of 12 months of home health experience within the past 3 years
  • Ability to travel within the assigned multi-county service territory

Preferred Experience / Skills

  • Experience with HCHB (Homecare Homebase) electronic medical records system
  • Familiarity working within a geographically assigned team-based care model
