Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established goals
Collaborate with interdisciplinary care team to ensure quality patient outcomes
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

Active Physical Therapist license in the state of California
Graduate of an accredited Physical Therapy program
Experience working in a skilled nursing facility (SNF) environment
Availability to work one weekend day as part of the regular weekly schedule

Preferred Experience / Skills

Prior contract or travel therapy experience
Familiarity with geriatric patient populations and SNF regulatory requirements
