Key Responsibilities

• Contribute professional and clinical knowledge as a member of an interdisciplinary therapy team to provide and manage patient care in a home health setting
• Provide occupational therapy services to patients primarily requiring short-term therapy following hospitalization or nursing home stays
• Travel within approximately 50 miles of the Fort Dodge area to conduct home visits, averaging 200–250 miles per week
• Document patient care using an electronic medical records system (EPIC)
• Participate in weekly virtual team meetings and monthly in-person team meetings
• Collaborate with on-staff therapy personnel including PT, PTA, and COTA team members

Required Qualifications

• Minimum of 2 years of occupational therapy experience
• Active Iowa state OT license
• Current BLS certification
• Valid driver's license and personal vehicle (mileage reimbursed)
• Must maintain a taxable address located more than 50 miles from the assignment location
• Must not have been employed by the facility within the last 12 months

Preferred Experience / Skills

• Prior home health experience (department is willing to train candidates without it)
• Experience with EPIC EMR
