Contract | Full-Time | Days (5x8 Hours, 8:00–17:00) | Field/Home Health | Onsite

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned geographic territory on the island of Oahu
• Complete a minimum of 25 patient visits per week to meet guaranteed hours requirements
• Conduct and complete OASIS assessments accurately and in a timely manner
• Maintain timely clinical documentation in compliance with agency and regulatory standards
• Travel independently between patient homes within the assigned coverage territory using personal transportation
• Participate in on-call rotation as required and complete all required orientation modules

Required Qualifications

• Active Hawaii state Physical Therapist license (pending license with eTA accepted if applied upon submission)
• Minimum of 1 year of Home Health experience
• OASIS experience required
• BLS certification through the American Heart Association (AHA) required
• Active, valid driver's license and access to personal transportation
• Commitment to timely communication and clinical documentation

Preferred Experience / Skills

• OASIS experience strongly preferred
• Candidates without Home Health experience will be considered if they hold outpatient physical therapy experience
• Familiarity with home health compliance standards and field-based care delivery
