Key Responsibilities

• Provide skilled physical therapy services to geriatric patients in a home health setting
• Complete OASIS assessments including start of care evaluations
• Manage a daily patient caseload of up to 6 patients while meeting productivity standards
• Travel to patient homes within the service area, with occasional visits requiring extended drive time
• Document patient care accurately and in a timely manner using HCHB electronic medical records system

Required Qualifications

• Active California Physical Therapist license required
• Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
• Proficiency with HCHB (HomeCare HomeBase) documentation system required
• OASIS experience required, including start of care assessments
• Current Basic Cardiac Life Support (BCLS) certification required
• Must have reliable personal transportation

Preferred Experience / Skills

• Experience working with geriatric patient populations in a home health environment
• Familiarity with productivity-based scheduling and point systems in home health settings
