Contract | Full-Time | Onsite | Days | 5x8-hour shifts, 40 hours per week

Key Responsibilities

Provide direct physical therapy services to patients in an outpatient setting
Evaluate, plan, and implement individualized treatment programs based on patient needs
Complete required clinical documentation accurately and in a timely manner
Collaborate with the care team to support patient progress and outcomes
Maintain compliance with credentialing and facility-specific requirements throughout the assignment

Required Qualifications

Active Physical Therapist license required
Experience in an outpatient physical therapy setting
Ability to meet contract credentialing and quality assurance requirements
Must obtain a permanent state license; compact license may be accepted at start but permanent licensure must be obtained promptly

Preferred Experience / Skills

Prior experience working in a high-volume outpatient or clinic-based therapy environment
Familiarity with travel or contract therapy assignment workflows and documentation standards
