Job Title

Physical Therapist (PT) - Home Health

Location

Paducah, KY

Schedule / Job Type

Contract | Full-Time | Onsite | Monday – Friday, Days (8:00 AM – 5:00 PM)

Key Responsibilities

Provide skilled physical therapy services to patients in a home health setting
Evaluate patients and develop individualized treatment plans to address functional deficits
Deliver therapeutic interventions aimed at improving patient mobility, strength, and independence
Document patient progress and clinical notes accurately and in a timely manner
Coordinate care with interdisciplinary home health team members

Required Qualifications

Active Physical Therapist (PT) license in the state of Kentucky
Graduate of an accredited Physical Therapy program
Ability to work a full-time Monday through Friday schedule

Preferred Experience / Skills

Prior home health experience strongly preferred
Experience with HCHB (Homecare Homebase) documentation software preferred
