Job Title

Physical Therapist (PT) – Home Health

Location

Marion, OH

Schedule / Job Type

Contract | Days | Monday – Friday, 8:00 AM – 5:00 PM | 13-Week Assignment

Key Responsibilities

Provide physical therapy services to patients in a home health setting
Complete OASIS assessments accurately and in a timely manner
Document all patient care activities using HCHB electronic medical records
Participate in on-call rotation as required
Meet productivity expectations of 30 points per week

Required Qualifications

Active Physical Therapist (PT) license in the state of Ohio
Minimum of 12 months of home health experience within the past 3 years
Proficiency with HCHB electronic medical records system
Experience completing OASIS documentation
Ability to fulfill a 40-hour per week commitment

Preferred Experience / Skills

Familiarity with home health regulatory and compliance standards
Strong time management and independent patient care skills in a field-based environment
