Job Title

Physical Therapist (PT) - Home Health

Location

Tulsa, OK

Schedule / Job Type

Contract | Full-Time | Days | Monday - Friday, 40 hours per week | Field/Home Health

Key Responsibilities

Provide skilled physical therapy services to patients in a home health setting
Evaluate and treat patients within an assigned territory in the Bartlesville region
Document patient care accurately and in a timely manner using HCHB electronic documentation system
Complete OASIS assessments as required for home health patients
Develop and implement individualized treatment plans to meet patient goals

Required Qualifications

Active Physical Therapist (PT) license in the state of Oklahoma
Minimum of 12 months of home health experience within the past 3 years
Proficiency with OASIS documentation
Experience with HCHB (Homecare Homebase) electronic medical records
Reliable transportation to travel within assigned territory

Preferred Experience / Skills

Familiarity with the Bartlesville, OK region
Strong organizational and time management skills for managing a home health caseload
