Physical Therapist – Home Health

Contract | Days (5x8 Hours, 8:00–17:00)
Key Responsibilities
- Provide physical therapy services to patients in a home health setting within an assigned territory
- Complete patient visits and maintain timely clinical documentation in accordance with compliance standards
- Conduct and complete OASIS assessments as required
- Participate in on-call rotation as needed
- Complete required orientation modules during onboarding
- Travel between patient locations using personal transportation, with mileage reimbursed at the IRS standard rate
Required Qualifications
- Active California Physical Therapist license (pending CA license considered if application is submitted prior to start)
- Minimum of 1 year of Home Health experience; candidates with solid outpatient experience may be considered
- BLS certification through the American Heart Association (AHA)
- Active, valid driver's license and access to personal transportation
- Timely communication and documentation practices are mandatory
- Completion of Home Health Checklist is required
Preferred Experience / Skills
- OASIS experience strongly preferred
- California license in hand at time of submission preferred
- Outpatient physical therapy experience considered in lieu of home health experience
