Contract | Days (5x8 Hours, 8:00–17:00)

Key Responsibilities

• Provide physical therapy services to patients in a home health setting within an assigned territory
• Complete patient visits and maintain timely clinical documentation in accordance with compliance standards
• Conduct and complete OASIS assessments as required
• Participate in on-call rotation as needed
• Complete required orientation modules during onboarding
• Travel between patient locations using personal transportation, with mileage reimbursed at the IRS standard rate

Required Qualifications

• Active California Physical Therapist license (pending CA license considered if application is submitted prior to start)
• Minimum of 1 year of Home Health experience; candidates with solid outpatient experience may be considered
• BLS certification through the American Heart Association (AHA)
• Active, valid driver's license and access to personal transportation
• Timely communication and documentation practices are mandatory
• Completion of Home Health Checklist is required

Preferred Experience / Skills

• OASIS experience strongly preferred
• California license in hand at time of submission preferred
• Outpatient physical therapy experience considered in lieu of home health experience
