Contract | Full-Time | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide occupational therapy services to patients in a home health setting across West Portland and surrounding areas
• Conduct patient evaluations and deliver skilled therapeutic interventions on a 1:1 basis, visiting up to 6 patients per day
• Complete Oasis assessments as required for home health documentation and compliance
• Utilize EPIC Remote Client (laptop) and Rover (phone app) for clinical charting and documentation
• Manage daily patient caseload efficiently, with awareness of travel routing to minimize drive time

Required Qualifications

• Active Oregon Occupational Therapist (OT) license
• Minimum 1 year of skilled home health experience
• Basic Cardiac Life Support (BCLS) certification
• Oasis assessment experience required
• Reliable personal transportation required for patient home visits
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client and Rover charting platforms
• Comfort managing a productivity-based caseload in a home health environment
• Experience with varied patient populations across a broad geographic service area
