Physical Therapist – Home Health

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Hood River, OR and surrounding areas
• Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
• Complete clinical documentation using EPIC Remote Client and Rover mobile application
• Conduct and complete OASIS assessments as required for home health patients
• Manage a daily patient caseload with travel routes grouped to minimize drive time; occasional radius of up to approximately 60 miles per day

Required Qualifications

• Active Oregon (OR) and Washington (WA) Physical Therapist licensure — both licenses required
• Minimum 1 year of skilled home health experience
• OASIS experience required
• Basic Cardiac Life Support (BCLS) certification required
• Primary COVID-19 vaccination series plus one booster required
• Must have reliable personal transportation

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for home health charting
• Experience meeting productivity point expectations in a home health setting
• Comfort managing a geographically distributed patient caseload with varying daily drive times
